Ensuring access to services
West Montreal Readaptation Centre (WMRC) has had a team devoted to Access services for many years. Led by Access manager Frank Vincelli, it has many responsibilities:
- receiving and evaluating all new requests for services
- conducting global needs evaluations for users and their families
- maintaining individual service plans (ISP) current
- management of the waiting list and setting priorities for service delivery
- producing statistics pertaining to the waiting list for services
- supporting users on the waiting list
- orienting all social emergencies
- acting as a liaison with other establishments requesting services
- managing inter-establishment transfers
- managing service contracts with other establishments
- conducting psychosocial evaluations and coordinating public and private curator dossiers
- offering psychosocial support to families
- recruitment and validation of residential resources
The Access team will be basing many of its future orientations on the Service Access Plan for Individuals with Disabilities recently released by Quebec's ministry of health and social services.
